Acne Mechanica: Breakouts From Helmets, Straps and Masks
Acne mechanica is caused by friction, pressure, heat and occlusion against the skin rather than by bacteria alone. It appears exactly where gear touches, in uniform small bumps confined to the contact area. Helmets, chin straps, headbands, masks, collars and pads are the usual causes. Reducing friction matters as much as treating the skin.

What acne mechanica is
Ordinary acne starts with a follicle blocked by sebum and dead skin cells, which bacteria then colonise. Acne mechanica starts with mechanical stress on the follicle.
Four forces act together where gear meets skin:
- Friction. Repeated rubbing irritates the follicle opening and damages the surface.
- Pressure. Sustained load compresses the follicle and disrupts normal shedding.
- Heat. Trapped body heat raises skin temperature and drives sebum production up.
- Occlusion. Nothing evaporates, so sweat and oil stay against the skin.
The result is inflammation of the follicle in a pattern that maps precisely onto the equipment. Bacteria are still involved, which is why clearing them helps, but friction is what starts it.
How to tell it apart from ordinary acne

The sharp boundary is the giveaway. If you can trace the outline of a chin strap or a helmet pad in your breakouts, it is acne mechanica.
Who gets it
- Cyclists and motorcyclists. Helmet straps and pads, for hours at a time.
- Footballers and rugby players. Chin straps, headgear, shoulder pads.

- Healthcare and trade workers. Masks, respirators, hard hats and hi-vis collars.

The fix
Treating the skin alone will not resolve it while the mechanical cause continues. You need both.
Reduce the mechanical load
- Loosen where you safely can. A strap tight enough to be safe is often tighter than it needs to be for comfort.
- Take it off between efforts. Helmet off between sets, cap off between rounds, mask off when you step outside. Brief breaks let heat escape.
- Add a moisture-wicking layer between gear and skin where possible, such as a thin skull cap under a helmet. Wash it after every use.
- Check the fit. Gear that moves rubs. Gear that is too tight compresses. Both cause it.
- Replace worn padding. Compressed foam concentrates pressure onto smaller areas.
Treat the skin
- Clean the gear. Helmet padding, chin straps, headbands and masks hold sweat and bacteria. Weekly minimum, more if used daily.
- Clear the contact area as soon as the gear comes off. Mist the area within 30 minutes, before the sweat film has had time to work. How to apply it.
- Do not scrub. The skin is already mechanically irritated. Adding abrasion makes it worse.
- Do not apply heavy products before wearing gear. Occlusion plus product is worse than occlusion alone.
- Skip harsh actives on the contact area while it is inflamed. Barrier-damaged skin under friction reacts badly.
