The more mundane explanation is timing. People usually start creatine at the same time as a new training block, which means more sessions, more sweat and more time in the post-workout window. The variable that changed was the training, not the powder.

Whey protein

Whey has a more plausible mechanism, though the evidence is still thin.

Dairy proteins can raise insulin-like growth factor 1 (IGF-1), and IGF-1 is associated with increased sebum production and follicular keratinisation. Some case reports and small observational studies have linked whey supplementation to acne in young men. That is real but it is a long way short of established.

Some practical distinctions if you want to experiment:

  • Whey concentrate vs isolate. Isolate has less lactose and fewer residual dairy components. People who suspect whey often tolerate isolate better.
  • Plant protein. Pea, rice and soy blends avoid the dairy pathway entirely. A clean test if you want one.
  • Sugar content. Many mass gainers and flavoured powders carry a significant sugar load, and high glycaemic load has better evidence for an acne link than the protein itself.
  • Total dairy. If you drink milk, eat yoghurt and take whey, the powder is one input among several.

Why supplements get blamed

The core problem is a delay. Bacteria multiply in the sweat film within about 30 minutes of finishing a session. The inflammatory response and the visible spot take another two to four days.

By Thursday, when the spot appears, you have trained twice more and eaten a dozen meals. There is no way to intuitively trace it back to Monday afternoon. So attention lands on the things you consciously changed, and for most people who train that means supplements.

How to actually test it

If you want a real answer rather than a guess, run it properly.

  1. Change one variable at a time. Cutting whey, creatine and dairy simultaneously tells you nothing about which mattered.
  2. Give it six to eight weeks. A skin cycle plus margin. Four weeks is the minimum to see anything.
  3. Photograph weekly in the same light. Memory is unreliable and skin changes are gradual.
  4. Count new spots, not total spots. Existing lesions resolve on their own schedule. New ones tell you whether the trigger is still active.
  5. Fix the post-training window first. Otherwise you are testing a supplement against background noise from a much larger variable.
